YOU WILL NOT OWE ANYTHING TO US OR TO THE LACTATION NETWORK UNLESS YOUR INSURANCE COVERAGE ENDS AND YOU FAIL TO NOTIFY US OF YOUR NEW COVERAGE. ANY BILLING RELATED QUESTION MUST BE DIRECTED TO THE LACTATION NETWORK.

ANTHEM + BCBS BEFORE THE BIRTH CONSULTS
BEFORE THE BIRTH: Comprehensive consultation an individualized plan for milk production or formula preparation and nursing, depending on your personal goals. You will understand how much to feed your baby no matter the type of milk you choose, latching, if and when to supplement and or pump in the early days. We will also measure your nipples to help size you appropriately for your pump flanges to ensure more effective and optimal milk removal.
ANTHEM + BCBS AFTER THE BIRTH CONSULTS
ONE BABY 75 MINUTES. TWINS UP 90 MINUTES. IF YOUR BABY(IES) IS/ARE NOT ON THE LACTATING PARENT’S POLICY, YOU MUST PAY A FEE PER CONSULT. IF ONLY YOUR BABY IS COVERED AND NOT YOU, YOU MUST BOOK A SELF-PAY CONSULT.
AFTER BIRTH INITIAL OFFICE CONSULT: whenever possible the initial after birth consult should be in person for the most optimal assessment. If you truly can not make it in person then contact us here to arrange for a tele-health booking. Comprehensive lactation and feeding assessment to assess latch, production, pump equipment and usage, oral restrictions like tethered oral tissues, health history that can impact production, clogged ducts, mastitis, over production, nipple pain or damage, bottle feeding, safe formula preparation. We also provide expert support for exclusive pumping, exclusive formula feeding, inducing lactation or re-lactation.
ALL FOLLOW UP REQUIRES A SCHEDULED CONSULT
ALL TELE-HEALTH CONSULTS ARE LIMITED TO THIRTY MINUTES
TELE-HEALTH DISCLAIMER: There are inherent limitations to a tele-health consultation because the provider will not be able to make an in-person physical assessment. There are potential risks to the technology used for tele-health, that are beyond the control of the provider, including interruption of connectivity, unauthorized access, and other technical difficulties with audio and or visual.

ONCE YOUR BABY IS BORN, WE HAVE TWO CLIENTS THAT WE PROVIDE SERVICES FOR THE LACTATION NETWORK WILL BILL FOR EACH THE PARENT AND EACH BABY IF THEY ARE ON THE SAME PLAN.
YOU ARE RESPONSIBLE FOR INFORMING US OF ANY INSURANCE COVERAGE CHANGES FOR YOU AND YOUR BABY
IT IS YOUR RESPONSIBILITY TO APPLY FOR PRE-APPROVAL PRIOR TO YOUR CONSULT
WE RESERVE THE RIGHT TO CANCEL YOUR APPOINTMENT IF YOU DO NOT APPLY FOR PRE-APPROVAL
IF YOU FAIL TO NOTIFY US THAT YOUR COVERAGE HAS EXPIRED, WE WILL BILL YOUR CARD ON FILE FOR THE FULL CONSULT FEE
CANCELLATION AND PAYMENT POLICIES
Appointments canceled with less than 48 hour notice will be charged the FULL consult fee which is not covered by insurance. ANY fees paid for services are non-refundable and expire 60 days after the original purchase. The payment and no refund policies are SEPARATE from the cancellation policy. Refunds will NOT be issued once you book. Each client is responsible for all fees associated with each consult. Payment is charged at the time the appointment is scheduled. By scheduling your consult, you grant us permission to communicate with your insurance company regarding the services provided to you and to your child(ren) and you agree to our payment policies.
Rental agreements: no refunds will be issued for any unused part of the rental terms. All equipment must be returned the day prior to the scheduled new charge in order to avoid being charged for a new rental term.
